Amesbury Soccer Association Spring 2009 Travel Registration. 

Thursday, Nov 6 & Nov 13 6:30PM -  8:00PM

You can register in person at registration night or by downloading the registration form at our website http://www.amesburyyouthsoccer.org/files/ASARegistrationForm.pdf and sending it with the appropriate fee, id picture (not needed if player was in 2008 Fall Travel) and birth certificate (only needed if player has never participated in ASA travel soccer) to:

1. The registration is for Boys and Girls:
U10 (at minimum: Grade 2, born on or before 8/1/2001. at maximum born on or after 8/1/1998)
U12 (Born on or after 8/1/1996)
U14 (Born on or after 8/1/1994)
U16 (Born on or after 8/1/1992)
U18 (Born on or after 8/1/1990)

2. Each applicant is required to provide:
1. Signed Registration.
1. Please list all previous Travel Soccer and Club Play experience.
2. Please identify primary phone number for contact purposes
2. Copy of player’s Birth Certificate
1. Only need if player has not played for ASA Travel in the past year.
3. Passport sized photo of player
1. Only needed if player was not in ASA Fall 2008 Travel soccer.
   
3. Registration Fee:
1. $80 for first player from each family.
2. Deduct $5 for each additional player from the same family.
4. Late Fee
1. Applications received after December 5th are subject to a $25 late fee.
5. Withdrawal - If any player decides to not play after his/her registration is received by ASA, refunds will be made under the following conditions:
1. Full refund if requested before teams are submitted to ECYSA.
2. Less $25, if requested after teams are submitted to ECYSA, but before 1st game of the Fall ‘08 Season.
3. No refund after the first game of the Fall ’08 Season.
6. Age groups U12 or greater may be subject to tryouts depending on the number of players who have registered prior to December 5th.   Tryouts and player assignments will be carried out as specified in ASA Operating Procedures.
