Fall 2008 Travel Registration:

1. Registration Nights at Cashman Elementary School (cafeteria):
   Wednesday, May 21, 2008 6-8 PM (This sign-up night corresponds with the 2nd Summer Soccer Sign-up night to be held at the same time and place, the Casheman Elementary School)
   Wednesday, June 4, 2008, 6-8 PM
2. Each applicant is required to provide:
1. Signed Registration.
1. Please list all previous Travel Soccer and Club Play experience.
2. Please identify primary phone number for contact purposes
2. Copy of player’s Birth Certificate
1. Only need if player has not played for ASA Travel in the past year.
3. Passport sized photo of player
3. Registration Fee:
1. $80 for first player from each family.
2. Deduct $5 for each additional player from the same family.
4. Late Fee
1. Applications received after June 10th are subject to a $25 late fee.
5. Withdrawal - If any player decides to not play after his/her registration is received by ASA, refunds will be made under the following conditions:
1. Full refund if requested before teams are submitted to ECYSA.
2. Less $25, if requested after teams are submitted to ECYSA, but before 1st game of the Fall ‘08 Season.
3. No refund after the first game of the Fall ’08 Season.

7. Tryouts will be held on June 17th and June 19th for U12 and older age groups.   Tryouts and player assignments will be carried out as specified in ASA Operating Procedures.
